The Query Wizard - Name and Description form lets you give your Query a name and a suitable description.  You must enter a name, but the description is optional.

Click on the 'Next >' button to move on.  If you are creating a new Query you will move on to the Query Wizard - Select System form, if you are modifying an existing  Query you will move on to the Query Wizard - Define Filter Conditions form.

Click on '< Back' to return to the Query Wizard - Introduction form.

You can click on Cancel (at any time) to leave the Query Wizard without saving your changes.
